Enrolment in the course is formalised by completing and accepting this form, and is finalised by paying the enrolment fee, as indicated by the commissioning party. Course enrolment can be made by paying a deposit or the entire tuition fee, enclosing an identity document and health card of the enrolled party.
The commissioning party undertakes to provide the training course once the minimum number of participants has been reached. It may also extend the deadline for submitting enrolments if it deems it appropriate.
The course venue is: Macko™ Academy – via Lagravinese 50/E, 70043 Monopoli (Bari) – Italy.
The participation fee includes: delivery of didactic material (electronic and/or paper), drawing materials and branded stationery; delivery of tattooing equipment on loan for use; issue of certificate of attendance.
Payment of the participation fee must be made no later than the working day prior to the start of the training activity (or as indicated by the commissioning party). Payment can be made no later than the deadlines set and/or communicated in one of the following ways
– bank transfer (bank commissions excluded) in favour of Macko Investment S.r.l, Bank: BPER Banca S.P.A, IBAN: IT49L05387415700002524548, BIC/SWIFT CODE: BPMOIT22XXX. A copy of the bank transfer must be sent to amministrazione@macko.academy or to +39 327 004 4066.
– cash and/or debit/credit card, at the Macko Academy headquarters in via Lagravinese 50/E Monopoli (BA)
– HeyLight instalment facility (Compass s.p.a.) through dedicated portals and/or at the Macko Academy premises in via Lagravinese 50/E Monopoli (BA) . However, in the event that the participant wishes to interrupt the use of the service once the course has started, he/she must still pay the full amount.
The Participant has the right to withdraw from the Contract, without penalty and without having to specify the reason, within 14 days from the starting date of the Course. For cancellations made within 48 hours prior to the course, there shall be no refund of the deposit or possibility to change the course start date. In order to exercise the right of withdrawal, the Participant shall send a written communication to the commissioning party, within the above-mentioned deadline, by means of: e-mail ( info@macko.academy) or PEC (mackoinv@pec.it). The withdrawal notice must contain: indication of the name and surname of the Participant; the name of the course, the wish to withdraw from the Contract, bank details for reimbursement. The commissioning party shall reimburse the Participant the full amount of the registration fee paid, within 14 days from the date of receipt of the withdrawal notice. The refund will only be made by bank transfer.

If the Participant interrupts the Course before the end, he/she shall be obliged to pay the participation fee in full and shall not be entitled to any refund. In particular and documented cases, the commissioning party may assess the possibility of partial reimbursement to the Participant, in proportion to the lessons not attended and/or the possibility of shifting the fee to another edition of the course.
The Institute reserves the right, due to organisational requirements, to modify the course programme, change the starting date of the course and replace the indicated lecturers with experts of equal professional standing.
Participants are required to
a) use the equipment provided by the commissioning party with the utmost care and diligence;
b) strictly adhere to the safety and conduct rules provided by the commissioning party;
c) not make integral recordings of entire lessons;
In the event that a participant commits the above violations, the commissioning party shall be entitled to remove and/or expel the participant responsible for the above violations from the courses and, if appropriate, to take legal action and/or complaints.The participant is obliged to behave correctly and respectfully towards property and persons. In the event of intentional damage to property, the member shall be liable for compensation. No credit or reimbursement will be given to customers expelled from classes for violating the code of conduct.
In the event of an accident occurring during the course in the classroom, Macko Academy is exempt from any liability. Macko Academy is exempt from any liability in the event of loss or theft on its premises and in the car park. Please do not leave personal items unattended.
The purchase of video courses (recorded or live) on Macko Academy dedicated portals implies acceptance of the terms and conditions of use specified at the time of purchase. As this is digital content, made immediately available for downloading or streaming, the provisions of current legislation on consumer rights in the European Union apply. According to Article 59 of the Consumer Code, the right of withdrawal is excluded for digital content provided in a way that is not supported by a tangible medium. Therefore, once the purchase has been completed, it will not be possible to obtain a refund.
The subscriber authorises the publication and dissemination of audio, video or photo material in which he/she is recognisable, free of charge and without time limits, in accordance with the laws in force. Material that offends dignity will be removed.
For Other declarations The undersigned declares: to be over 18 years of age. to have at least 10 years of schooling, i.e. to have completed at least the second year of high school. Pursuant to art. 46 of D.P.R. no. 445/2000, aware of the responsibilities and penal sanctions provided for by art. 76 of the same D.P.R. for false statements and mendacious declarations, under my own responsibility, the undersigned declares that what is reported in this curriculum vitae corresponds to the truth.
